If you are interested to become a member of Hyack Swim Club you can register in person at:
The following items need to be completed when registering:
- Family Registration form
- Athlete Registration form
- Medical form for each swimmer (click here to download and print in PDF format)
- Code of Ethics forms for each swimmer - Signed by parent & Swimmer (Provincial Group and above)
- 1 Start-up fee cheque (Family Registration Fee, Uniform/Equipment orders, Swim BC fees, Cash Calendars)
- 11 Monthly cheques
- Uniform / Equipment order form (click here to download and print in PDF format)
- Volunteer sign-up sheet

- Fundraising information
- Club & Policy guidelines
- Group description
- Proposed Meet schedule (for Provincial to Senior National groups only)
